Every fatality investigation report MSHA has published from 2003 to the present. Across all years since 2000, 1,163 of 1,210 recorded fatalities are matched to their report.
The 47 that are not fall into three stated groups: reports MSHA has not yet published for recent accidents, early reports from 2000 to 2002 still being recovered, and one 2016 fatality for which MSHA never published a report. Each is itemized.

MSHA found that mine management had decided to stop installing berms near open stopes, and that management travelled through the area daily. The report also found that a backup camera, broken for years, contributed. It prints two different figures for how long the berm was missing and does not reconcile them.
A telehandler the crew was relying on was removed eleven minutes before a plate fell. The crew chief told investigators he did not know the final bolts were out. MSHA cited the operator and the contractor in parallel, under the same two standards, and made no equipment-defect finding.

The report gives two different figures and does not reconcile them. The discussion of workplace examinations says the missing berm was a hazardous condition that existed for approximately one and a half months [MSHA Report of Investigation, p. 5 (PDF p. 7)]. The enforcement actions for the berm say the condition existed for approximately two months [p. 7 (PDF p. 9)].
The record supports both statements. It does not say which is correct.
